Course Description
Incident Investigation VR 6.0 is an up-to-date course that will train workers and supervisors to determine the causes of an incident and enact corrective measures to prevent a recurrence.
The course is designed to provide the knowledge, methodologies, and skills necessary to systematically and comprehensively analyze and understand workplace incidents or accidents.
Promptly reporting and investigating incidents can reduce subsequent incidents and protect your equipment damage and workers from further harm.
It is important to have at least one worker trained in the process of incident investigation including how to prepare and conduct the investigation, how to distinguish between direct and root causes, and how to develop recommendations to correct any deficiencies and prevent a recurrence.
This course qualifies for 0.3 BCRSP CEUs for the Board of Canadian Registered Safety Professionals (BCRSP) – Continuing Professional Development Program (CPD Program) for the purpose of recertification in maintaining the BCRSP certification.
Course Outline
Incident investigation is a process of gathering facts and drawing conclusions. The course covers the following topics:
- Purpose and benefits of incident investigations,
- Description on how to conduct an investigation,
- Recognition and distinction between direct causes of incidents and the root (underlying) causes
- Development of recommendations for
corrective actions in order to prevent incidents from recurring.
Certificate of Completion:
Upon successful completion of this online course, a certificate of completion will be available to download and print.

Approximately 2.5 hours
Pass Mark
A mark of 80% must be achieved in order to receive a certificate of completion.
